Please enter particulars of emergency Cases with Critical Dx / Concerning / Conflicting findings encountered during 24 Hour duty in this 4C Register. The concerned faculty along with another senior faculty will review the same, next day morning between 9 am to 9:30 am and clarify the findings. About 2 to 3 such cases (either CT/MR/X-ray/USG) may be entered per duty. If it is an ultrasound case, please upload small videos also.
On submitting the brief form below, the duty Asst will be auto-intimated by mail with CC to Dr Kalaivani, who shall supervise this implementation the next day.
